Patients come first at Crystal Lake Health Center.  We provide and coordinate comprehensive services for the entire family.

Our five sites and twelve providers allow easy access for all your medical needs.

PLEASE READ - You will not be able to use the link to your "My Health Portfolio" on this page UNTIL after you receive verification that your new account is active.

You will initially receive an email inviting you to activate your account. This is the only way you can sign up for your account.  If you have not received this email, it is because we do not have your email address in our file.  Please contact Keisha at 231-882-1037 and give her your email address.  You will respond to the email you received by clicking the link in the email and completing the required information.  It may take up to 24 hours for your account to be activated (requests sent on Friday will be activated on Monday morning).  Once your account is activated, you will receive another email with the new username you requested.  You will then be able to access your account from this page.
